Deadlines

Original signed forms are due to the Office of the Secretary of State no later than May 1st (May 3rd for 2010 because the 1st falls on a Saturday).

On or before April 23, 2010:

        • All forms should be mailed to the University Ethics Office.

After April 23, 2010:

        • Mail your Statement directly to the Office of the Secretary of State to avoid late filing penalties; AND
        • Mail a photocopy of the original (marked "COPY") to the University Ethics Office for their records.

If you miss the May 1st(3rd) filing deadline with the Office of the Secretary of State, you will be penalized. See the General Questions and Criteria for Completion section of this site for specific details.

All forms submitted on or before April 23, 2010 should be returned via US Mail** to the University Ethics Office at:

**For UIC and UIUC employees, there is no campus mail to the Ethics Office, so you MUST place your form in US Mail.
