Deadlines, Mailing Details and Penalties
Deadlines
Employees required to file a Statement of Economic Interests (Statement) with the Office of the Secretary of State must have the document post-marked by no later than May 1, 2019. This is an absolute deadline, after which, financial penalties ensue.
Mailing Details
Before April 17, 2019, to satisfy the requirement within the law that all forms be reviewed by the University Ethics Officer prior to filing, please place your form in US Mail to:

Please DO NOT send your form to the University Ethics and
Compliance Office after April 17, 2019. Forms not received in our
office by May 1, 2019 will be considered late by the Office of the
Secretary of State.
UIS employees can place their forms in campus mail. UIC and UIUC employees must send their documents via US Postal Mail as there is no campus mail to or from UIS.
After April 17, 2019, to ensure timely filing with the Office of the Secretary of State, please mail your form directly to:

Employees who mail their original form directly to the Secretary of State should send an image of their form, clearly marked “COPY” to ethicsofficer@uillinois.edu.
Penalties
Forms filed between May 1 (May 2nd when the 1st falls on a Sunday) and May 15 (May 16th when the 15th falls on a Sunday) will be assessed a late fee of $15. After May 15th (May 16th when the 15th falls on a Sunday), any form filed will be asses the flat $15 rate, in addition to a $100/day charge for as many days late as the form is received. For example: If a form is received by the Office of the Secretary of State May 10th, the filer will be fined $15. However, if the form is not received until May 20th, the filer will be fined $515. These fines are assessed to individuals only and must be paid personally by the delinquent filers to the Office of the Secretary of State.
