Though the law requires ethics officer review of Statement of Economic Interests forms prior to filing with the Secretary of State, beginning today, April 15, 2016, employees should mail their forms directly to the Office of the Secretary of State.  DO NOT MAIL YOUR FORMS to the University Ethics and Compliance Office.  Instead, place original, signed documents in the mail to:

Office of the Secretary of State
Index Department, Ethics Section
111 E. Monroe
Springfield, IL  62756

Employees should then send a copy of their form, clearly marked, "COPY - Original to SOS" to the University Ethics and Compliance Office for review (email: ethicsofficer@uillinois.edu; fax: 217-206-6211).
